The early start is the whole point of this departure. Grutas Tolantongo is 201 km and about 3 h 50 min by road from Mexico City on the operator's distance table, and the caves, tunnel, waterfalls and swimming pools inside the park run only from 8:00 to 17:00. Leaving the city between 4:20 and 5:00 am puts you at the entrance around the time those sections open, so the cave and the tunnel are quiet when you walk in.
Crowding is predictable. A park co-owner has put normal weekend occupancy at 20-30%, long holiday weekends at about 75% and the main days of Semana Santa at 100%, which is why the terrace pools look very different at 8:30 am and at 1 pm. You are already on the road home by then.
On temperature the park contradicts itself, and it is worth knowing before you pack. The cave, tunnel and terrace pools are published at 36-38 °C; the river is given as 34-36 °C on the park's English page and 36-38 °C on its Spanish one. The water emerging inside the mountain is about 37 °C and turns tepid once it meets the cold river outside the grottoes.
Bring cash. Nothing inside the park takes cards, including the restaurant, the shop and the lockers, and the nearest ATM is in Ixmiquilpan, 42.9 km and about 1 h 11 min away. Budget for breakfast, lunch, a locker at MXN 150 with MXN 50 refunded when you return the key, and MXN 300 if you want the zipline circuit, which is not part of this tour.

Meeting point: if you are outside the pick-up zone, a meeting point is coordinated for you and confirmed the day before, usually around Roma or near the Angel de la Independencia. You need to be there by 4:30 am.
Pickup: pick-up and drop-off are offered in Roma Norte, Roma Sur, Condesa, Juárez, Polanco, Anzures, Cuauhtémoc, Tabacalera and Centro Histórico. If you are staying in Centro Histórico, drop-off is near the Palacio de Bellas Artes, an area with heavy traffic and restricted access when streets are closed or events are on.
